Model with a dagger between the teeth, pin closure on the back, minted, model with "all the teeth." There are, in fact, variants of skulls in which some teeth are missing.

As evidenced by numerous period photos, it was worn on size 42/44 caps (the single-button ones), on the shoulder straps of the BBNN, and on the chest ribbons.
